NOVEMBER 21 TOX EVENT — BOOKING TERMS

• Reservation and pricing: A $50 deposit reserves your scheduled appointment and is applied toward your treatment. Event pricing is $10.25 per unit for treatment received at this event. Your prescribing provider determines the appropriate treatment and dosage. The remaining balance is due on the day of treatment.

• No-shows and cancellations: If you do not attend your scheduled appointment, your $50 deposit is forfeited and will not be refunded, transferred, or credited toward a future appointment. Cancellations made less than 48 hours before your appointment also forfeit the deposit, subject to the medical exceptions below. Cancellations received at least 48 hours beforehand receive a deposit refund.

• Good Faith Exam required: Your Good Faith Exam with our authorized prescribing provider, medical clearance, and required paperwork must be completed at least 48 hours before your appointment. Completing a questionnaire alone does not constitute medical clearance. If you fail to complete these requirements after being offered a timely exam, your appointment will be canceled and your deposit forfeited.

• Medical eligibility and exceptions: Booking does not guarantee treatment. Our prescribing provider may decline or postpone treatment when medically appropriate. If you are determined to be medically ineligible, your $50 treatment deposit will be refunded. Your deposit will also be refunded if we cancel your appointment or cannot arrange the required exam in time.

• Accurate health information: Please disclose all medications and supplements, allergies, medical conditions, pregnancy or breastfeeding, recent toxin treatments, and any illness or infection. Notify us of any changes after your exam. Do not stop prescribed medication without instructions from your prescribing clinician.

• Appointment readiness: Clients must be 18 or older and bring photo identification. Please arrive on time; late arrivals may not leave enough time for safe treatment. Clients impaired by alcohol or other substances will not be treated.

• Consent and follow-up: Separate treatment consent is required after discussing risks, benefits, and alternatives. Results vary and are not guaranteed. Additional treatments or touch-up units are not automatically included. Any follow-up charges will be explained before treatment. Follow your provider’s aftercare instructions and promptly report concerns.
